Form 990, Part VI, Section B, Line 11b The 990 form is placed in the shared google drive, available to all board members and reviewed by the officers of the board, particularly the treasurer. All financial documents are available for review and oversight by all board members, and are periodically reviewed by the officers of the board. All conflicts of interest are examined and decided by majority rule of the board, and then declared at least annually and noted in the minutes. Any changes during the year are also noted in the minutes. Any director with a conflict of interest refrains from deliberation and voting on any items impacted by the noted conflict. The fact that a conflict was identified and that the director refrained from deliberation and voting is also noted in the minutes. Form 990, Part IX, Line 11g Other fees for services include: Operations Manager (Program $51,867), Security (Program $11,793), and Miscelleneous ($3,845 Program and $1,344 Management and General). During the COVID 19 pandemic, the organization lost most of its revenue and was forced to drastically downsize its staff, including its only bookkeeper. The return was originally due by 11/15/22. An extension was filed to extend the due date to 5/15/23. Due to the storms in California, the IRS extended the due date to October 2023, then again to November 2023. Still, the organization needed more to compile the records needed to file the return. The return was prepared as of 5/7/24. However, because of the fiscal year change in the previous year and the late filing of the current return, the tax preparer's Intuit tax preparation software was not capable of transmitting the return to the IRS. Data had to be moved to a new software program that was capable of transmitting, resulting in an additional delay of several weeks.
