The parent supplies their income level and number of depentds on the application. The student enrollment is then verified with the administration of the school by our organization. Verification is also made to ensure that the student is not receiving tuition funds from and Empowerment Scholarship Account (ESA) through the Arizona Department of Education. Residency of the student is verified with the information supplied on the application and awards are only available to Arizona residents. An application is only valid for one school year, and the parent must re-apply each year. The Yuma Education Scholarship Fund board of directors reviews the application and awards scholarships based on financial need. Upon board approval, the scholarships are issued to the qualified private school in Arizona. The school is instructed by a letter that they may not receive an amount per student9from all tuition organizations) in excess of the total tuition amount due for each school year. In the event an excess amount is received by the school, then they are instructed to return the excess funds to the school tuition organization in a timely manner. The application is available on our website in an electronic submission format. Form 990, Part VI, Section A, Line 8b We maintain only one board of directors and not any individual committees. Form 990, Part VI, Section B, Line 11b Form 990 and all supporting schedules were completed by the Executive Director and then provided to the President of the organization during a monthly meeting for review. All financial information used to complete the form 990 is reviewed each month by the board members during the monthly meeting. Form 990, Part VI, Section B, Line 12c The board will discuss any conflicts presented during the monthly meeting and determine the appropriate course of action as the event is brought to attention. No conflicts have been discussed to date. Form 990, Part VI, Section B, Line 15 The board established a compensation plan for the Executive Director during the monthly meeting held in August 2008. The compensation plan states that the Executive Director shall be paid a percentage of the total donations received during the month. During the monthly board meeting in October 2022, the board voted to modify the compensation to a flat amount of $50,000 for the year that will be paid out evenly each month. An additional commission would be calculated at year end should $50,000 be less than 3% of the annual revenue. Form 990, Part VI, Section C, Line 18 Form 990 will be made available for public view if a formal request is submitted to the board of directors. Form 990, Part VI, Section C, Line 19 The organization keeps all governing documents, policies, and financial statements at the office located at 550 E 32nd street, Suite 3 in Yuma Arizona. Request by the public to see any of these documents would be received by the Executive Director and taken to the board for advisement, and then made available in paper form. This is not a late filing of our Form 990. An application for Automatic Extension of Time to File an Exempt Organization Return, Form 8868 was filed and accepted on November 15, 2023. Notice CP211A dated December 11, 2023 was also received from Department of Treasury.
