Patients seen by the Low Vision Program at the Eye Institute require a refraction and magnification as part of their evaluation. RMLEIF pays the $75 fee for people on a limited income who can't afford it. Patients need not be Lions. The grants are for specific individuals who are identified on the check from RMLEIF by their Medical Records Number (MRN). 56 75 0 RMLEIF pays the provider $75 per patient REFRACTION AND MAGNIFICATION PROGRAM, MAGNIFICATION ONLY. Patients seen by the Low Vision Program at the Eye Institute require a refraction and magnification as part of their evaluation. If a patient has already had a refraction, only a magnification is needed at a cost of $45. RMLEIF pays this fee for people on a limited income who can't afford it. Patients need not be Lions. The grants are for specific individuals who are identified on check from RMLEIF by their Medical Records Number (MRN). 8 45 0 We pay the provider with a $45 check PATIENT ASSISTANCE PROGRAM. RMLEIF helps pay for transportation, lodging, and meals for needy patients and a family member from Wyoming or Colorado while the patient undergoes examination and/or treatment at the Institute or elsewhere because necessary vision care is not available where the patient lives. Patients and family members need not be Lions. The program is administered though local Lions Clubs who evaluate the financial need of the patient and family and contact RMLEIF to request aid. Receipts are required for verification of expenditures. 1 1000 0 Amount of cash provided Schedule I, Part I, Line 2 (1) REFRACTION AND MAGNIFICATION PROGRAM. Patients seen by the Low Vision Program at the Eye Institute require a refraction and magnification as part of their evaluation. The $75 fee for these services must be paid by uninsured patients, is not covered by basic Medicare or Medicaid, and sometimes is not covered by other forms of insurance. When a patient schedules an appointment and lacks insurance that covers this cost, the Institute's scheduler says to the patient: "The Lions Club has set up a fund for people who are on a limited income and can't afford to pay the fee. If you would like, I can send a request to see if they're able to cover the cost." If the patient responds "yes", the response is recorded on the patient's record by Institute staff and RMLEIF accepts the patient's statement that he or she cannot afford this cost, believing that it is not reasonable, given the modest amount of assistance, to verify patient income or other measures of patient financial need. RMLEIF pays for a refraction and magnification with a separate check on which Institute staff write the patient's medical records number and the date of service. The cancelled checks substantiate RMLEIF's payment for service, the date service was provided, and can be traced to the patient's medical record for further information if necessary. RMLEIF relies upon the internal control procedure at the hospital to assure that these payments are used for the purposes intended by RMLEIF. (2) PATIENT ASSISTANCE PROGRAM. RMLEIF facilitates access to care for people in financial need by helping to pay for transportation, meals, and lodging while the patient receives care. Local lions clubs to make a determination of financial need and assist in administration of the program. All expenditures in excess of $500 must be verified with receipts. The conference was hosted by CUHealth's low-vision program at the Rocky Mountain Lions Eye Institute. Form 990, Part VI, Section A, Line 6 All members of Lions Clubs in Colorado and Wyoming who are in good standing with Lions Clubs International are automatically members of RMLEIF. Form 990, Part VI, Section A, Line 7a The Trustees of RMLEIF are elected by the members of Lions Clubs International in their respective Colorado or Wyoming districts to a three year term, with a two consecutive term limit, but are eligible for re-election to another term after a two year interruption. If a Trustee leaves office before conclusion of his or her term, or the Trustee position is open for another reason, the Governor of the Lions in that Trustee's district appoints a replacement Trustee for the remainder of the term. Form 990, Part VI, Section B, Line 11b Form 990, Part VI, Section B, Line 11b - In accord with RMLEIF's bylaws, the Finance Committee ensures that all required reports are filed with the Internal Revenue Service and makes the forms available for the Board of Trustees (RMLEIF's governing body) to review. This Form 990 was prepared by the Treasurer and reviewed in detail by the Treasurer and Assistant Treasurer prior to filing. A complete copy of the final Form 990 was sent via email to all Trustees prior to filing. Form 990, Part VI, Section B, Line 12c RMLEIF Trustees, Officers, and Advisors must review and sign RMLEIF's conflict of interest policy each year, which emphasizes the importance of the policy to everyone and refreshes knowledge of the policy. RMLEIF had one material transaction during 2023 that required application of the policy: the printing and mailing of our annual letter to members. There were 4 bidders for this job and the low bid was selected, which came from a business managed by a RMLEIF Trustee. The rules in the policy were properly applied.
