This is an annual activity and is conducted in Orlando, FL at a facility suitable for such event. The fraternity spends about 20% of its time on scholarship activity. There is a committee that uses an application review process and monitors the use of scholarship funds provided to the recipients. Scholarships are funded from funds raising activities such as Golf, Gospel Program, membership dues and public donations. Due to covid these fundraising events were postponed. Form 990, Part VI, Section A, Line 7a The Orlando Alumni Chapter have dues paying members who have the right to vote in the members of the governing board. This vote occurs each year. Form 990, Part VI, Section A, Line 7b The Orlando Alumni Chapter have dues paying members who have the right to vote on any decisions that are brought forth by the governing body. Form 990, Part VI, Section B, Line 11b The governing body is notified that the 990 form is available upon request for review and made aware of its filing. Form 990, Part VI, Section C, Line 19 Our organizations financial records and governing documents are made available upon request to the governing body. Form 990, Part XI, Line 9 The Orlando Alumni Chapter has an administration building and land where monthly chapter meetings are held. Also, the Kappa League Guide Right program hold their monthly meetings as well. Monthly committee and board meetings are held in the administration building. Book value less depreciation, less remaining total due on mortgage. Letter sent to IRS on 7/11/22 Reference: Returned 990 EZ for tax period 10/1/19-9/30/20 From: Nathaniel Stephens Dear Internal Revenue Service: I was given the assignment to fix the reference problem and prepare a complete and accurate Form 990 for tax period 2019. At the time of the assignment, I was in rehab at Orlando VAMC 13800 Veterans Way, Orlando FL 32827. I am a retired accountant. We have had turnover of membership during the pandemic and research and relocating financial files have been a problem. However, we began the research and relocating effort in the week of May 16. My last rehab scheduled treatment was June 15 after which I would be free to devote more time to Form 990 preparation. On the 12th of June I came down with a cough and was advised to get tested for Covid and I complied. Despite having all four required immunizations, I was tested on the 14th and covid confirmed on the 15th. I was confined to isolation for 5 days. After 5 days of isolation, I wasn't feeling well and still had a cough. However, the VA said I was well enough to resume normalcy. I am approximately one third complete on the Form 990. On the morning of June 29, I passed out in the bathroom and was carried to the ER at Orlando Health Ocoee Hospital by first responders. While there i underwent surgery for removal of blood clot from my lungs and around my heart. Doctors informed me that I was close to death when I arrived to the hospital. I was released from the hospital for bed rest and recouperation Friday July 1. On Monday July 4th, 2022, I was in the ER at the Orlando VA Medical Center for treatment of Hematoma/Bruises resulting from prior surgery and on July 5, 2022, I was admitted to the ER for Chronic Pain. As of the date of the letter, I feel much better and have returned to work on the Form 990. Mr. Marvin A. Poole erroneously filed the Form 990EZ for tax period instead of Form 990 and is no longer in this department. We have worked diligently and expeditiously t0 prepare a complete and accurate Form 990. As stated earlier, we have had turnover of personnel during the pandemic and some of our finance records were being sought for. Your cooperation is appreciated.
