Form 990, Part VI, Section B, Line 11b The conflict of interest policy of the organization is discussed with all employees on a yearly basis The organization has an external accountant that prepares form 990 and applicable schedules, after completion the form is handle to the Executive Director and she discuss it with the Board of Directors Form 990, Part VI, Section B, Line 12c On a yearly basis the organization request members of the Board of Directors and employees to disclose if there is any conflict of interest Form 990, Part VI, Section B, Line 15 Election of the Executive Director is a Board of Directors process, they have an onboarding and compensation process. Form 990, Part VI, Section C, Line 19 The organization upon request will provide the governing documents, conflict of interest policy, and financial statements available to the public during the tax year. Last August 1, 2022 we received a letter from the IRS, reference 0422604607 LTR 2694C. The letter informed us that the IRS can not process the Form 990 and was returning the form to us; the letter received did not include any enclosure. We apologize for any inconvenience, please note that we had different situations due to inability to work on site due to Covid 19 restrictions here in Puerto Rico. On the other hand we did not have a good technological infrastructure that help us file our form online. To be responsible we file an extension to file and on a timely manner filed the form on paper
